Thank you for choosing Demong Associate Eyecentre for your upcoming surgery. We want to ensure your surgical experience is as smooth as possible. Toward that goal, this section provides copies of the instruction sheets you have been given at the time of patient counseling. We also provide streaming media so you can review the whole process again in the comfort of your own surroundings.

WE WANT TO HEAR FROM YOU! We strive to meet your expectations to the best of our abilities and appreciate your feedback at critical times during your surgical process. To facilitate that, we have also provided two surveys:

• a PreOperative Survey to be completed after your initial consultation but before your surgical experience
• a PostOperative Survey to be completed within the first week after your surgery has been completed.
