PreOperative Evaluation
If you have already scheduled surgery with Dr. Demong or Dr. Chow, and his nurse has informed you that you require a Physical Examination and Lab Tests in preparation for that appointment, these forms can be printed and taken to your family physician at your earliest convenience. Instructions for returning the information to us are included in the cover letter.
