   Demong Associate Eyecentre requires minimal identifying information to respond to your requests for information. Demong Associate Eyecentre then becomes the sole owner of information collected through the Internet. No information is released to, or shared by, any outside party. The information you provide is used solely to deliver information or services specifically requested by you.

2. Information that is Personally Identifiable (General Inquiries)

   Our registration format requests certain contact information about you. We require the following:

   • Your name
   • Your email address
   • A telephone number where you can be reached during the day

   By forwarding this information from our Contact Page, you consent to us using the information to contact you to deliver information or services as requested.

3. Email responses to Your Requests

   Electronic mail is considered official correspondence of Demong Associate Eyecentre and as such, email correspondence that occurs between you and Demong Associate Eyecentre is maintained in a confidential file on our internal server. Your name and phone number and/or email address may be cross-referenced to your patient file if you become a patient of Demong Associate Eyecentre and all correspondence may become part of the permanent patient record.

   1. Use of Information

      Demong Associate Eyecentre collects demographic and other statistically relevant information as well as details of your business hours and resources to deliver information, products, or services only as specifically requested by you. We also use contact information from the registration format to send you information about us and when otherwise necessary. Such information may also be used to assess your satisfaction with us and obtain feedback regarding the services we provide.

      We may use the contact information to send you information, such as news bulletins and newsletters, by email or other means which we believe will be of value to you. Associates may opt-out of receiving future communications at any time: see the section regarding "Opt Out"

      Finally, the information we collect will be maintained in a file that we will review in a variety of other circumstances, such as enforcing our Terms of Co-management, analyzing and understanding our network associates and their needs, and for the administration and provision of services.

      No personally identifiable information, personal correspondence, or proprietary information will be shared with any other third party, except with your expressed permission or at your specific request.

   2. How to Opt Out or Contact Us

      You can review and request that we modify or remove the information you provide us in the registration process if this information changes. In the event you no longer wish to receive email communications from us, you may opt out by contacting us at marketing@demong.com

    

5. Security

   Demong Associate Eyecentre employs standard industry measures to protect and ensure the confidentiality of your personally identifiable information and correspondence, including the use of firewalls. These security measures are inclusive of, but not exclusive of, the following:

   • Physical security of the technology systems against theft and damage
   • Software integrity to ensure that only authorized personnel can implement or modify software or view personal communication
   • Disaster recovery planning processes and systems
   • Data integrity to check the validity of certain data
   • Data security administration procedures to ensure proper monitoring and auditing. Email may be audited by authorized managers of Demong Associate Eyecentre for any violations of law, breaches of clinic policies, or for communications harmful to the clinic.

   We cannot guarantee absolute security of information, given the current state of the Internet so we recommend that highly confidential or sensitive information not be sent through email correspondence. Patient history or physical examination information should be conveyed only by secure, encrypted communication through a parallel email system currently (06/04) under development, not through the website.

   We provide links to manufacturers from this website to provide more detailed information about our technology. We are not responsible for the privacy policies and practices of third parties to which we provide links from this website. Demong Associate Eyecentre does not have control over the privacy policies applied by any other party that you may disclose information to when linking from this site.
